Create a New Violations Form
Also known as: add a violations form, set up a new violation notice form, create a compliance/covenant form
Use this guide to add a new violations form so your community can receive violation reports and input.
Step 1: Create the Form
- Click on the Forms tab on the vertical Dashboard menu at the left side of the screen.
- Click on the Violations tab.
- In the VIOLATIONS window, click +Add New Violations Form.
- In the ADD NEW VIOLATIONS FORM window:
- Enter a name for your form in the Form Name box.
- In the Submission Response box, type or paste the message members see after submitting the form.
- Enter the email address of the person who should be notified each time someone fills out this form, in the Form Recipient 1 box.
-Click the red Remove link to delete a recipient. (Every form must keep at least one recipient.)- Activate Make Form Uneditable if you want to block anyone from changing the form once it has received at least one submission. (This cannot be undone by anyone, any desired changes require a brand new form to be built)

Click Save to create the form, or click Cancel, press Esc, or click the X to exit without saving. After saving, the FORM MANAGEMENT window opens automatically so you can build out the form's fields.
Step 2: Build the Form Sections
Violation forms organize their input fields into sections. Each section can hold full-width fields (spanning the whole section) or half-width fields (two side by side).
- In the FORM MANAGEMENT window, click +Add New Section to create the form's first section.
- In the ADD SECTION window:
- Activate Show Title if you want the section name visible when the form loads.
- Type the section name in the Section Name box.
- Click Save Section to create it, or Cancel to discard.
- Click +Add Half-Width Input or +Add Full-Width Input to add a field to the section.
- To remove a section, click the red trash-can icon next to the section name (next to the blue "Edit" pencil icon).
To edit an existing violations form's sections

Click Forms > Violations on the Dashboard menu.

In the VIOLATIONS list, find your form.

Click the three-dots menu in the ACTIONS column and choose Form Builder to reopen FORM MANAGEMENT.
Step 3: Choose Field Types
Click +Add Half-Width Input (or +Add Full-Width Input for Spacer, Paragraph, Upload, or Date Picker fields) and choose from these field types:
- Short Text Input — Brief text entry (e.g., a house number or one-line answer).
- Set a Title and optional Placeholder example text; activate Required if the field must be filled in; click Save Text Input Field.
- Long Text Input — A paragraph-length answer (e.g., a description).
- Set Title, Placeholder, and Required as above; click Save Paragraph Text Field.
- Dropdown Menu — Members pick one value from a list.
- Enter a Title, then type each choice into an Option box (two boxes appear by default — click Add Another Option for more, or remove to delete one). Activate Required if needed; click Save Dropdown Menu.
- Checkbox — Members can select one or more of several pre-set options.
- Same setup as Dropdown Menu; click Save Checkboxes.
- Radio Buttons — Members can select only one of several pre-set options.
- Same setup as Dropdown Menu; click Save Radio Buttons.
- Spacer — Adds blank visual space between fields. No settings to configure.
- Paragraph — Adds static instructional text to the form (not a fillable field).
- Enter your text in the Title box; click Save Paragraph Text.
- Upload Field — Lets the person completing the form attach a document.
- Enter a Title; activate Required if needed; click Save Upload Field.
- Date Picker Field — Lets the person completing the form select a calendar date instead of typing free text.
- Enter a Title and optional Placeholder; activate Required if needed; click Save Date Picker Field.
For any field-type window, click Cancel, press Esc, or click the X to close without saving.
Step 4: Edit, Remove, or Reorder Fields
- To edit a field, click the edit link next to its name.
- To remove a field, click the remove link next to the edit link. (Spacers have only a "remove" option since there's nothing to edit.)
- To reorder a field within its section, click and drag the four-headed arrow icon to the left of the field's name.
Step 5: Save and Finish
- When you're happy with the form, click Save Form, then click the back to violations link under the "FORM MANAGEMENT" title to return to the VIOLATIONS window.
- To close without saving, click Cancel, press Esc, or click the X.
- To switch to editing a different form, choose it from the Select Form to Edit dropdown and click Select. If you have unsaved changes, a LEAVE PAGE? prompt appears — click Continue to switch and discard changes, or Cancel/Esc/X to stay on the current form.
