Customize or Edit a Violations Form

After you create a violations form, use the Form Builder to add, remove, or rearrange the fields members and admins fill out on it. To change the form's name, recipients, or "Members Only" setting instead, see Edit the Settings on a Violations Form.

Steps

  1. Click the Forms tab on the vertical Dashboard menu at the left side of the screen, then click Violations.
  2. Locate your form in the list. In the ACTIONS column, click the three-dots menu and choose Form Builder.
  3. On the FORM MANAGEMENT page, use the controls on any existing field or section to make your changes (see below).
  4. To add a new field, click Add Half-Width Input or Add Full-Width Input at the bottom of a section, then choose a field type in the ADD CUSTOM FIELD window.
  5. When you're finished, click Save Form. Click Cancel instead to discard your changes.

Working with Sections and Fields

  • Click Add New Section at the bottom of the form to start a new group of fields.
  • Each section has edit and remove links next to its heading — use edit to change whether the section title is shown, and remove (trash icon) to delete the whole section and its fields.
  • Each field also has its own edit and remove links.
  • Drag a field's move handle (the four-way arrow icon to its left) to reorder it within or between sections.

Available Field Types

When adding a new field, choose from:

  • Short Text Input — a single-line text box for brief answers.
  • Long Text Input — a multi-line text box for longer answers.
  • Dropdown Menu — lets the person filling out the form pick one option from a list you define.
  • Checkbox — lets them select one or more options.
  • Radio Buttons — lets them select exactly one option from a list.
  • Spacer — a blank visual divider with no settings of its own.
  • Paragraph — static instructional text you write, not a fillable field.
  • Upload Field — lets them attach a file (such as a photo of the violation).
  • Date Picker Field — a calendar-style date selector.

Note: Marking a field as required means a submission can't be completed until it's filled in — use this for the information you truly can't proceed without.
