Customize or Edit a Form
After you create a custom form (see Build a Custom Form), use these steps to change its basic settings or the fields members and visitors fill out.
Step 1: Open the Form's Actions Menu
- Click the Forms tab on the vertical Dashboard menu at the left side of the screen.
- Click the Custom Forms tab.
- In the CUSTOM FORMS window, locate your form in the list.
- Click the ⋮ (three-dot) menu in the ACTIONS column for that form's row. This opens a dropdown with several options, including Duplicate Form, Access Management, Edit Form Settings, Form Builder, Download CSV (only if the form has at least one submission), and Deactivate.
Step 2: Edit Basic Form Settings
To change the form's name, recipients, or type:
- From the ⋮ Actions dropdown, click Edit Form Settings.
- In the FORM SETTINGS window that opens, update any of the following:
- Form Name
- Submission Response — the message the person who filled out the form sees immediately after submitting
- Form Recipient — click Add Another Recipient to notify more than one person by email when a submission comes in
- Make Survey/Poll, Enable Ticketing, Members Only, Make Form Uneditable, Member Dashboard/Portal Accessible checkboxes
- Click Save to accept your changes and return to the CUSTOM FORMS window. To cancel instead, click Cancel, press Esc, or click the × in the top right corner.
Note: Turning Make Survey/Poll or Enable Ticketing on for an existing custom form moves it to the Voting/Surveys or Ticketing System tab right away. Turning Enable Ticketing back off on a form that's already become a Ticketing form does not move it back to Custom Forms — it stays listed under Ticketing System regardless of the checkbox. If you need a plain Custom Form again, build a new one instead of trying to convert an existing Ticketing or Survey form back.
Step 3: Add or Edit Sections and Fields
To add new input fields or change the layout of the form itself:
- From the ⋮ Actions dropdown, click Form Builder instead of Edit Form Settings. This opens the FORM MANAGEMENT window with that form pre-loaded.
If you open the wrong form by mistake, use the Select Form to Edit dropdown at the top of the window to switch to the correct one — it also includes a + CREATE NEW FORM option.
Custom forms display their requests for input in individual sections (for example, "Personal Information" and "Reason for Outreach" on a contact form). Each field within a section has edit and remove links next to it.
- To add a field to an existing section, click Add Half-Width Input (two fields side by side) or Add Full-Width Input (one field spanning the row) under that section.
- In the ADD CUSTOM FIELD window, choose the field type: Short Text Input, Long Text Input, Dropdown Menu, Checkbox, Radio Buttons, Spacer (a blank space, useful for lining up fields), Paragraph (static instructional text with no input), Upload Field (lets the person attach a file), or Date Picker Field.
- Click Add New Section at the bottom of the form to create an entirely new group of fields.
- When you're done, click Save Form. To discard your changes instead, click Cancel.
Note: This same Form Builder also powers Voting/Surveys and Ticketing System forms — the field types and section layout work identically no matter which type of form you're editing.
