Create a Forms Page and Attach a Violations Form

Building a violations form (see Create a New Violations Form) only creates the form itself — members can't submit it until it's attached to a page on your public website. This article covers publishing that page.

Steps

  1. Click the Manage Site tab on the vertical Dashboard menu at the left side of the screen, then click Site Setup. This opens the Site Builder.
  2. Click Add Page under the section of your site menu where you want the new page to appear.
  3. In the NEW PAGE window, set Page Template to Forms.
  4. Fill in a Page Name (the Page URL fills in automatically, but can be edited) and any other settings you want, then click Add.
  5. On the new page, hover over the empty form widget and click Select Form.
  6. In the EDIT FORM window, open the Select a Form dropdown and choose your violations form.
  7. Click Update to save your choice, or Cancel to back out without changes.
  8. Click Save and Publish at the top of the Site Builder to make the page live.

Note: The Select a Form dropdown lists every custom form your organization has built — violations forms, plain custom forms, ticketing forms, and surveys all appear together in the same list. Double-check you've chosen the right one by name before clicking Update.
