Create a Custom Forms Page and Attach a Custom Form

Building a custom form (see Build a Custom Form) only creates the form itself — members and site visitors can't access it until it's attached to a page on your public website. This article covers publishing that page.

Steps

  1. Follow the "Add a Page" instructions in the Site Setup help topic to create a new website page.
  2. When you reach the Page Template step, choose Forms from the dropdown menu instead of a standard content template.
  3. Continue through the rest of the Add a Page process (naming the page, choosing its location in your site navigation, and publishing it).
  4. On the new Forms-template page, hover over the empty widget area and click Select Form.
  5. In the EDIT FORM window, open the Select a Form dropdown. It lists every form your organization has built — Custom Forms, Voting/Surveys, and Ticketing System forms all appear together in one list, so double-check you're choosing the correct one by name.
  6. Choose your custom form, then click Update.

The form now displays and accepts submissions on that page.

Tips

  • If you want the form limited to logged-in members only, confirm the "Members Only" setting on the form itself (in Customize or Edit a Form) rather than trying to restrict it at the page level.
  • A single custom form can be attached to more than one page — attaching it to a second page doesn't remove it from the first.
  • If you don't see your form in the Select a Form dropdown, confirm it hasn't been deactivated — deactivated forms drop out of this list until reactivated (see Deactivate a Form).
