Overview

Custom Forms let you collect information from members or the public for use cases beyond dues and violations — think contact forms, maintenance requests, architectural change requests, or general comment forms. Go to Forms > Custom Forms to build one.

The same builder also powers Voting/Surveys and the Ticketing System — turning on the right option while building your form is what makes it a survey or a ticket-tracked form instead of a plain custom form (see "How does the ticketing feature work?" and "Online Voting and Surveys Overview" for those specific flows).

How to Create a Custom Form

  1. Go to Forms > Custom Forms.
  2. Click Add New Form.
  3. Fill in the required fields:
    • Form Name — the internal title for your form.
    • Submission Response — the email address(es) that receive a copy of each submission.
    • Form Recipient — who is notified when a new submission comes in. Click Add Another Recipient to notify more than one person.
  4. Choose which options apply to this form:
    • Make Survey/Poll — turns this into a Voting/Surveys form instead of a plain custom form.
    • Enable Ticketing — turns this into a Ticketing System form, so each submission is tracked as a ticket with a status.
    • Members Only — restricts submissions to logged-in members.
    • Make Form Uneditable — locks the form so residents can't edit an already-submitted response.
    • Member Dashboard/Portal Accessible — makes this form appear as a widget option on the Member Dashboard.
  5. Click Save to create the form, or Cancel to discard it.

Once saved, you'll still need to attach your new form to a page on your public site or member portal before residents can access it (see "How do I create a ticketing page on the public side of the site?" for the general page-attachment pattern, which applies to any form type).

Managing Existing Forms

The Custom Forms list shows every form you've built, with columns for Form Name, Type (Standard, Survey, or Ticketing), Submissions count, Modified date, and row Actions for editing or managing each form. Check Show Inactive to include forms you've deactivated in the list.

Use the Search Forms field to find a specific form once your list grows.
